There are two forms to fill out. The 3949 (which you send to Fresno): https://www.irs.gov/pub/irs-pdf/f3949a.pdf

Then the 211, which is the whistleblower reward form: https://www.irs.gov/pub/irs-pdf/f211.pdf

My suggestion is to send the 3949 with supporting documentation. Send the 211 in with a copy of the 3949. Mail it all at the same time and sit back and wait years.

I was audited three years ago.

The IRS agent all but said that they only have enough people to go after low hanging fruit. In my case, it was their lack of knowledge about the industry I worked in, and how expenses were classified.
