How does something like this happen?

The warning has to be given by a military official. You just don't make a mistake. Or if you do, you have to explain it by now. And the Military official has to explain why he made the mistake.

So whatever story they give will be a lie. Trump might have just done this to take attention off him being a racist.

The Hawaii folks never got a warning. It got sent by accident, sort of like how you have to be careful to hit Reply All instead of Forward when you tell somebody that an email you both got was full of shit.
