Then if you take a $100K out for production/help costs, $1,700,000 is left, and then the only subtraction would be his airtime charge, which is probably about $500K-$1M, I would guess (anyone help on this?). $1.8M - $1M = $800K (most conservative estimate).

So, low-end he books $800K, High end he books $2.5M.

Malpractice premiums for doctors are high. For other professions like lawyers and real estate agents, even though you are dealing with multi-million dollar transactions, MP for 1-2M of coverage is only about $200-250/mo.
