The problems with those 2 Navy ships were endemic to 7th Fleet surface operations. Basically the safety culture was non-existent and CO’s were taking ships out minus the requisite number of qualified watch standers. This meant that you had OOD’s who were responsible for the operation of the ship who had maybe gotten 3 or 4 hours of sleep in the 48 hours prior to the accidents. In the weeks leading up to the collisions when people noticed the unsafe practices, they were basically told by superiors “you can sleep when you’re dead.” In fact, the Navy just brought up both CO’s and several other officers on charges including negligent homicide as a result of the investigation. There was no hacking on those ships, just CO’s who were more concerned with looking good for the DESRON commanders at the expense of safety. The fact that both collisions occurred with ships in DESRON 15 is no accident. It was a result of the practices encouraged under that command. The charges brought against the CO's are almost unheard of in such a situation, but the investigations unearthed such astounding lapses in safety that they became necessary. Just adding things I've heard from friends still in the fleet. |
