When Hans Gruber got thrown off the building in Die Hard. Alan Rickman hated heights and even though he was only going to be dropped like 20' onto an airbag, he was scared and agreed to only one take. The director told him he would count to 3 and then drop him. He only counted to 1, so Rickman's reaction is genuine.
Kevin Spacey throwing the plate of food at the wall in American Beauty was also unscripted.
