Best case scenario you can use him with men on and no one out. You may be able to hide him in the field for 1 inning for a second at bat. Perhaps if CJ is in the 5th or 6th inning. I want no part of him defending without a strike out pitcher.

What does being medically cleared even mean? Is something like that simply a non-starter? Or can madden go against Dr's orders.

Drs orders means if they play him, the union is going to sue the hell out of the Ricketts...

If he had been cleared for limited play, the only scenario I saw was if he led off an inning when they were down 2-3 runs, and they sent 7 guys to the plate. Then maybe you could hide him in the outfield for an inning, tell him to do absolutely nothing except run on a perfectly straight line, and basically play with 8 defensive players and take your chances. Then let him PH the next inning and get him the hell out of there.
