I'm no professional umpire, but it sneaks up on you. Like when I ump high school games the catcher usually frames the outside pitch pretty well and you get a consistent zone. Drop down a few levels and on an outside pitch the catcher is usually moving their body to the ball and taking it farther out of the zone every time, but since you're looking for it you can still have a good zone. Now when a HS catcher all of a sudden botches the frame you kinda get mixed up. Now hopefully I'd never get so mixed up I'd call that Lackey pitch a ball, but who knows.

As an ump you should be judging/calling the ball where it crosses the plate, not on where the catcher manipulates it with his glove. That said, perhaps it was a bit much of me to wish death by cancer on today's umpire.
