From what I can tell in limited research its about split half with people who say you should tip the same 15-20% and those who say less.

That's why you just tip 15% on the whole bill. It takes care of itself.

Everyonce in a while when I leave a tip on my card after a I pay a bill, the amount never gets debited from my card. Just the original meal price gets charged.

Its like getting as with your debit card. It shows the original meal price and then a couple days later the tip gets added. Like when you get gas with your debit it takes 1 dollar and then a few days later the rest gets added.

I pay on the card and tip in cash. Its the best thing for the server. We all know they are cheating on their taxes anyway, so might as well give it to them than a % to the owner who is charging you $48 for that bone-in.

Cash doesn't always help. The place my wife works at the owner takes 5% of sales regardless. So a big tab + little tip kills them.
