Define healthy. Salads can even be unhealthy when they are slobbered in dressing and include egg yolks and cheese. Wendy's probably has the closest thing to healthier fast food with baked potatoes and some other options where the food has not been sitting in lard.

Chipotle chicken bowl. Brown rice, omit cheese and sour cream if you want to get rigid. Their ingredients are pretty good, healthwise, but you end up undoing all the benefits with a 700 calorie white flour tortilla, or white rice. Taco Bell has a bowl now too, but I think they only have white rice. Grilled chicken breast anywhere is a good option, just watch out for or minimize empty carbs and sauces.

I always do that at Chipotle except I have black beans and I leave the cheese. That fat soaked tortilla is a wreck for one to eat.

Salad without salad dressing isn't very tasty. When you order fast food you should acknowledge that the stuff is bad for you and just get a kid's meal or just a sandwich and skip on the drink. People have different ways of looking at the nutritional content, but it's mostly a juggling act of sodium, saturated fat, carbohydrates and calories. The problem is also that for the most part there is no fiber in the fast food. With what they charge for fruit at fast food places you're better off just going to a cheap produce market and grabbing the fruit before you go out the door.

Meat is tasty, but it isn't particularly good for you. Even eating lower-fat meat at Subway can predispose you to getting colon cancer.
