She is a DB, for sure. Some of you are being too nice.

The reason is, everybody makes social faux pas now and then, but over time, you learn from them. This is a grown ass adult, who has accrued enough life experience to have learned that you don't interrogate strangers about their medical situation on a damn elevator. That's pretty basic stuff. So, this woman is either willfully ignorant, or has simply decided that her personal definition of tact is better than everyone else's. She didn't walk away feeling bad, because, she has most certainly had similar conversations before, and hasn't stopped initiating them. She's a DB.
